COVID-19 Protocol and Contract for Participation

Thank you for choosing to ride with Rim Tours for your mountain bike adventure! In this new time of the COVID-19 Pandemic, we are adjusting our practices and implementing procedures to allow for your participation in our tour and as such are asking you, our guest, to agree to certain protocols and exchange of information so that we can both be comfortable with this trip and your participation therein. If you do not agree to the below, you will not be permitted to participate in our tour. For the purposes of this document, “Tour Activity” shall mean the Rim Tours mountain bike tour for which you are registered to participate.

Please read each statement carefully and initial that you understand and agree to the statement.

1. I hereby certify that to the best of my knowledge I and any minor family members or other minors who are participating in the Tour Activity, am or are not currently infected with the Coronavirus/COVID-19. I certify that I nor any minor family members with me do not have any of the following symptoms of Coronavirus/Covid-19: sore throat, fever, headache, shortness of breath, cough, diarrhea, GI issues, muscle aches and pains, decreased or lack sense of taste or smell.

2. If I or any minor family members or other participating minors with me have previously been infected with the Coronavirus/Covid-19, I will provide evidence of the date of a positive result and/or a document from my doctor certifying that I am currently healthy to participate in this activity and am not in danger of infecting other participants or staff.

3. I further certify that in the past 2 weeks I and any minor family members or other participating minors with me, have not been in contact with or exposed to any person suspected or known of being infected with the Coronavirus/Covid-19 and that I have taken proper precautions to ensure this is true.

4.I agree to provide for myself and any minors participating with me my/our own mask or face covering to fully cover the mouth and nose. I agree to provide a mask or face covering for each of my family members that are participating with me. I further agree to provide any other items of personal protection equipment you feel is necessary to limit or prevent the contracting of COVID-19. (Rim Tours will provide hand washing and sanitizing materials)

5. I agree to strictly follow the directions of Rim Tours’ staff and wear the above-mentioned mask when asked to do so, and to abide with any other COVID-19 health protocols implemented by Rim Tours during the Tour Activity. I understand that failure to follow directions for wearing a mask and abiding with any other health protocols will result in the end of my participation in the Tour Activity. I also agree that any and all minor family members accompanying me in this Tour Activity will also follow directions of Rim Tours’ staff and wear their own mask when asked to do so.

6. I further agree that I, and minors participating with me, shall follow all social-distancing guidelines as required by Rim Tours, the state of Utah, Federal Land Agencies and any other governing bodies as to the COVID-19 pandemic having jurisdiction over me for this Tour Activity. I understand and agree that if I/we do not follow such guidelines, I/we shall not be permitted to participate or continue to participate in the Tour Activity

7. I agree that as a condition of participating in this Tour Activity, I and any minors participating with me may be asked to have my temperature taken with a no-touch device and I agree in advance to this request. I agree that if my temperature or any minor’s temperature participating with me exceeds 100.4, I/we/they will not be allowed to participate in this Tour Activity.

8. I certify that I have been vaccinated for Covid-19 on the following dates: (Note vaccination is not a requirement for participation)

9. I acknowledge that in the event that I have been vaccinated for Covid-19 (or Not), I will still follow all health protocols referenced above for the entire length of the tour.

10. Last, I acknowledge and agree that I have had ample time to review the within document prior to my execution thereof, understand the terms hereof, and sign it voluntarily.
